A photo remembers the label. A reviewed equipment record carries the source, full identifiers and the questions that remain open.

A number needs a unit, circuit, source and time. Missing or stale inputs should remain visible, and derived values need eligible direct inputs.

A PDF alone is not a delivery receipt. Keep visit evidence, author, version and review state together while treating office receipt as its own step.
